B'NAI ZION TIME SHEET TO SUBMIT HOURS FOR PAY

Use this form to submit hours to be paid by B'nai Zion for work you have already done. 

*You must have a W-9 on file to be paid by B'nai Zion. Depending on your job, there may be other paperwork that must be submitted as well.

**Note that B'nai Zion typically writes checks in the middle and the end of the month. 
Write in the type of job, such as: Security, Custodian, Substitute Teacher, etc.
Put total number of hours. If there is a minimum for your job, please put that number even if you actually  worked fewer hours.
Enter amount you are paid per hour.
Enter total amount you believe you will be paid. This amount is subject to confirmation and may be reduced by taxes, etc.
