We would appreciate if you take a moment to complete this form that gives us information about yourself including what name will be used on your certificates and prayer book, and so that the congregational leaders who will be presenting you with gifts will have some things as background for your presentation. (For example, they can say that they have known your family for generations, and know that you love reading… or whatever.)

Please write your name the way you want it to appear on the prayerbook and certificates that we give you. For example, you may want just your formal first and last names, you may want first and middle and last names, or you may want the name you like to be called plus your last name - but no middle name, etc. 
Include your parent's Hebrew names. Rabbi Jana can help with how it is spelled in Hebrew.

Please explain whether you would want the B/Mitzvah's name online, parents' names online, etc. We can give minimum information (only say "Bar or Bat Mitzvah Service") in our event lists or as much detail as you feel comfortable sharing online for the event. Check with the Rabbi for details.
