3rd Student Registration Form
Use this form to register a third child for Religious School, Hebrew Class, or Chai 5.

Most of the details about the family have already been submitted with the registration of the first child. If you have two or more children to register, please make sure that you fill out the 1st child registration form for one child. If you have more than three children to register, please use the appropriate (4th, 5th child) registration forms for the additional children.
